Chestfield Dental Practice welcomes new patients who wish to register, and patients who need a one-off or emergency visit.
- New patients are asked to arrive at least 10 minutes early to complete a digital medical history form, and to bring details of medical conditions and medication.
- Children under 16 need to be accompanied by an adult.
- X-rays are charged in addition to the new patient fee.
- If you cannot attend, please give at least 48 hours’ notice where you can, so the time can be offered to someone else.
- Casual emergency visits are booked with a fee paid in advance by card over the phone. That fee covers the visit. Further treatment that cannot be finished in the same appointment may cost more.
- Denplan and NHS treatment are available where they apply. Ask reception on 01227 793 799.
